USPS 1583 Form

The USPS 1583 Form is required by the USPS for us to be able to receive your mail and parcels. Although it may look intimidating, we don't need every section filled out. Go ahead and fill out sections 1, 5 (your email address), 6, all of the 7s, and a signature at the bottom right(number 16). When you've finished that, go ahead and scan it and send it back to us!
